VISION
Provided by AIG. This is an elective benefit paid 100% by the employee. Premiums for Vision: Employee Only: $10.88/Month; Employee-plus-partner: $19.06/month; Employee-plus-child(ren): $19.06/month; Family: $27.24/month pretax.
PRESCRIPTION DRUG PLAN
Provided by Anthem. Co-pay per 30-day prescription is $15/generic; $40/preferred; $60/brand; 30% of cost for injectable drug. Mail-order program is available for maintenance drugs at 2X co-pay for a 90-day prescription.**
FLEXIBLE SPENDING ACCOUNT
Pre-tax plan available for medical/dental premiums, unreimbursed medical expenses and daycare and/or transportation expenses. Employees are eligible from their date of regular full-time or part-time (minimum 20 hours/week) hire.
401(k) RETIREMENT PLAN
Immediate eligibility and immediate vesting of funds contributed after 1/1/2001, if working a minimum of 20 hours/week. Employee contribution up to 100% of earnings, up to the IRS maximum. PMS matches 100% up to 3% of earnings, 50% from 4-5% of earnings. Net PMS match is 4% with 5% employee contribution. Investments offered through Mass Mutual.
LIFE & A.D.D. INSURANCE
Equivalent to twice the annualized base salary at no cost to the employee up to a maximum of $200,000. Premiums paid 100% by PMS. **

TUITION REIMBURSEMENT
Currently available to employees after three months of regular full-time (minimum 30 hours/week) employment prior to starting the course, or for part-time employees, upon completion of 5 years of continuous employment (minimum of 1040 hours/year) prior to starting the course.

Accrue at rate of
15 days (120 hrs.) during first year,
16 days (128 hrs.) after 1 year of service,
17 days (136 hrs.) after 2 yrs.,
18 days (144 hrs.)/3 yrs.,
19 days (152 hrs.)/4 yrs.,
20 days (160 hrs.)/5 yrs.,

HOLIDAYS
Regular employees are currently eligible to receive 10 paid holidays per year. Pro-rated for regular part-time employees.
PROFIT SHARING
Regular full-time and part-time employees of Particle Measuring Systems may be eligible to participate in the Particle Measuring Systems Profit Sharing Plan if one is authorized for the plan year.
